STUDENT ENROLLMENT

Required documentation for new students:

• State Certified Birth Certificate
• Proof of Residency (Driver's License, utility bill, rent bill, etc.)
• Immunization Record
• Enrollment/Emergency Card*
• Record Request*
• Internet Agreement*
• McKinney Prescreen* 

New kindergarten students also need:

• Vision Screening
• Hearing Screening
• Health Appraisal,* signed by a doctor
• Kindergarten Readiness Checklist*
• Parent's Report*

Optional forms:

KINDERGARTEN ROUND-UP:

Algonac Community Schools has an all-day, every-day kindergarten program.

Fair Haven will be having our kindergarten round-up on May 11, 2012.  Please call (586) 725-7911 to make an appointment to bring your child to attend the round-up.  Appointment times are 9:15 a.m., 10:30 a.m. and 12:30 p.m.

During round-up the children will meet their teacher and participate in some typical kindergarten activities.  Parents will attend an informational meeting where the school principal will answer questions regarding Fair Haven and the all-day program. The St. Clair County Health Department will also be here to administer vision and hearing screenings (a requirement to enter kindergarten.)
